RESOLUTION OF THE CITY COUNCIL OF THE CITY OF CHULA
VISTA APPROVING THE FIRST AMENDMENT TO THE AGREEMENT
BETWEEN THE CITY OF CHULA VISTA AND THE COUNTY OF
SAN DIEGO FOR THE CITY TO PROVIDE SENIOR INFORMATION
AND REFERRAL SERVICES TO THE SOUTH BAY AREA THROUGH
SEPTEMBER 30, 1982
The City Council of the City of Chula Vista does hereby
resolve as follows:
NOW, THEREFORE, BE IT RESOLVED that that certain first
amendment to the agreement between the City of Chula Vista, a
municipal corporation, and the County of San Diego, for the City
to provide senior information and referral services to the South Bay
area through September 30, 1982, dated the 11th day of
May 1982, a copy of which is attached hereto and incorporated
herein, the same as though fully set forth herein be, and the same
is hereby approved.
BE IT FURTHER RESOLVED that the Mayor of the City of Chula
Vista be, and he is hereby authorized and directed to execute said
agreement for and on behalf of the City of Chula Vista.

. AMENDMENT TO AGREEMENT
This
First
AMENDMENT TO AGREEMENT is made by the COUNTY OF SAN
DIEGO herein called "County," and City of Chula Vista
, herein called "Contractor"
WHEREAS. County and Contractor have previously entered into an Agreement, a copy
of which is on file with the Clerk of the Board of Supervisors as Contract No.
17693

WHEREAS, County and Contractor desire to amend said Agreement to provide for
continuation of services, NOW THEREFORE, County and Contractor agree as follows:
1. That the purpose of this Agreement is to provide services as set forth in
the attached Service Provisions, Exhibit C, which is by this reference
hereby incorporated herein
2 That this Amendment will become effective on July 1,1982, and extend
through September 30, 1982.
3 That Contractor recognizes that County expends Older Americans Act funds
on a fiscal year basis, and therefore Contractor expressly agrees that it
is to receive its funding on that basis, and that the obligation of the
County under this Agreement is severable for and limited to the amount of

The South Bay Information and Referral Center is a Senior Infor~tiop.
and Referral program that services clients sixty (60) years of age
and above. The Center provides bili~gual (Spanish and English) information
agency referrals, follow-up, resource development and update, linkages,
and outreach to the elderly of 'the South Bay area.

Section 3. The Goal of the Project is:
To effectively mat~h the needs .of the elderly in South Bay with
existing services and resources in the community;

Section 4. The Objectives of the Project to be accomplished during the
contract period are:
1. To provide information and referral services to 1,300 South Bay elderly
by effectively matching the needs for serVices with existing resources.
2. To provide arrangements and appointments to link 600 clients with
appropriate community. services through effective referral.
3. To provide evaluation/follow-up to 450 elderly clients to determine the
quantity and effectiveness of the referral to existing resources.
4. To increase th; availability of vital resources for the elderly in
South Bay by maintaining an up-to-date resource file of 400 agencies
and organizations.
Contractor will be required to submit a quarterly status report on objectives.

Section 5.
A separate section 5. must be completed for each Program. A separate Program
must be identified for each subsection of a Title. Title Ill-B. III-Cl. and
III-C2 are separate subsections. Fo"rms COA 186, 187 or 188 (as received in the
Contractor's Negotiation Package) must be submitted each month (unless waived)
for appropriate Programs and forwarded to the Area Agency on Aging by the 10th
day-of the month for the previous month~s services. "

Because the needs of each service area may vary during the term of this
Agreement, service levels may be modified by written approval fro~ the County's
Representative in response to Contractor's written request to adjust the above
projections. Appropriate ~ustification will be required.
Section 6.
In addition to the units of service in Section 5, the Contactor may also
provide the following services as a part of the total program, however, NOT
MORE THAN 10~ OF THE OLDER AMERICANS ACT RESOURCES MAY BE DEVOTED TO THESE
SERVICES.
