PURPOSE:
The purpose of this procedure is to outline the steps and responsibilities of each
discipline when plan checking, processing and inspecting projects requiring special
inspection,
Note: Throughout this procedure, HERS (Home Energy Rating System) raters are to be
considered special inspectors and the procedures outlined below apply to HERS
raters as they apply to special inspectors unless noted otherwise,
PROCEDURES:
California Building Code, Section 1701, and California Energy Efficiency Standards,
require special inspection for the following types of work:
Special inspection may not be required when the work requiring special inspection is
performed on the premises of an approved fabricator and the procedure for "Shop
Fabrication" is complied with, See Items F (a) and (b) under "Inspection,"
During Plan Check:
On projects where the Plans Examiner identifies work requiring special inspection
and/or shop fabrication; the Plans Examiner shall do the following:

A. Identify all work requiring special inspection and request that the designer shows
the information in one location on the plans (i.e, Title/Cover Sheet, S-1 Sheet-
Special Inspection Table/Schedule,)
B. Insert a comment in the correction list stating that the attached "Property
Owner/Contractor Agreement Form" to be completed and signed prior to permit
issuance,
C, Attach the applicable special inspection forms to the correction list (for HERS,
"Property Owner/Contractor Agreement Form" and energy compliance forms CF-
4R and CF-6R.)
0, Complete the "Special Inspection" screen in Permits Plus,
At Building Counter:
The Development Services Technicians (DS1's) will hand over or fax to the applicant
the special inspection forms that are attached to the correction lists generated by the
plans examiners.
At time of permit issuance, the DST must collect a completed "Property
Owner/Contractor Agreement Form" and attach it to the Permit Application,
Inspection:
Prior to the first day a special inspector is scheduled to be on the job site, he/she must
do the following:
1, Notify the Inspection Section of the permit number, project address, date and
approximate time he/she is scheduled to be on the job site, and a phone number
that he/she can be reached at (preferably his/her cell phone,) This can be
accomplished by either calling in the information or completing Form 4545 and faxing it
to the Inspection Section,
2, Fax a copy of his/her qualifications, experience and driver license/photo 10 to the
Inspection Section,

The two requirements shown above are also required when a replacement special
inspector, other than the originally approved special inspector, takes over the project on
a temporary or permanent basis.
Special inspectors can notify the Inspection Section by phone (619) 409-5868 or Fax
(619) 216-7306,
The following are the steps taken by the Inspection Section staff:
A, Staff receives a call or a fax from a special inspector or Inspection Agency
identifying the permit number, project address, date and time the special
inspector's first day on site,
ß, The Office Specialist or Development Services Technician enters the information
on the "Special Inspection" screen in Permits Plus,
C, The Office Specialist or Development Services Technician forwards the
information collected under item (A) above to the Building Inspector (BI) assigned
to the project.
0, The BI incorporates the time into his daily schedule and meets the special
inspector at the job site to verify his/her qualifications, inform him/her of the City's
procedures and expectations and give him/her the "Special Inspection
Instructions To Speciallnspectors/lnspection AgencieslTesting Labs/HERS
Raters."
Special Inspector Qualifications
Special inspectors shall have current copies of certificates or cards for work requiring
special inspection and a current driver license/photo 10,
Except for grading, smoke control, and other specialty inspections for which a
certification is not required by law, special inspectors must be certified in their areas of
expertise. The following table indicates required certification for each type of special
inspection:

1. The soils engineer or Geotechnical engineer of record may designate a qualified
individual as their agent to perform special inspection on grading, excavation and filling.
Note: Except for grading, smoke control, and other specialty inspections for
which a certification is not required by law, the fact that a person is the
architect or engineer of record should be no factor in determining whether the
person is qualified as a special inspector, If the architect or engineer is
qualified as a special inspector by demonstrating satisfactory competence in
a particular type of construction, it is acceptable for the architect or engineer
to perform special inspection for a project.
E, Special inspection reports and test results are to be reviewed by the BI assigned
to the project within two working days from the date of receipt of the report or test
results. Identified issues/concerns are to be communicated to the special
inspector or Testing Agency by the next working day from the date the
issue/concern is identified,

F. The BI shall collect the following forms, if applicable, at the stage of construction
as identified below:
a, "Application to Perform Off-Site Fabrication" - Form 4540: This application
shall be completed by the fabricator of building components when such
components are fabricated within the premises of the fabricator's shop,
This application must be submitted to the Inspection Section for approval
at least three business days prior to the scheduled start of fabrication, A
BIIII (or Snr. BI) shall review the application within the next working day of
receipt for completeness and qualification, If the application is not
complete or the fabrication shop is not qualified, the BIIII (or Snr. BI) shall
immediately notify the fabrication shop and the contractor of the
requirements,
b, "Certificate of Compliance for Off-Site Fabrication" - Form 4541: This form
shall be completed by the fabricator of building components when such
components are fabricated within the premises of a fabricator's shop, This
form must be filed with the Inspection Section and approved prior to the
installation of the fabricated components,
c, "INSTALLATION CERTIFICATE" (CF-6R) and "CERTIFICATE OF FIELD
VERIFICATION AND DIAGNOSTIC TESTING" (CF-4R): Form CF-6R to
be completed and signed by the installer and form CF-4R to be completed
and signed by the HERS rater. Both forms must be submitted to the
Inspection Section prior to "Final" inspection,
d, "Special Inspector/Inspection Aqencv!Testinq Laboratorv Final Report"-
Form 4543 (not applicable to HERS raters): This form shall be completed
by the special inspector and/or the special inspection agency and/or the
construction materials testing laboratory after all work requiring special
inspection or testing have been completed, and submitted to the
Inspection Section prior to final inspection,
e, "Property Owner's Final Report" - Form 4542: This report shall be
completed by the property owner or his/her authorized agent of record or
the architecUengineer of record acting as the property owner's agent for
the types of work requiring special inspection, construction materials
testing or off-site fabrication, This form must be filed with the Inspection
Section prior to final inspection,

Prior to "Final", and only after the 81 verifies that all required reports and forms have
been submitted and are in compliance, he/she must approve Item 4470, "8ldg, Special
Insp," in the Permits' Plus Inspection Module,
Quality Control:
A. The 81 shall visit the job site at least once a week to verify that the special
inspector is performing his/her duties while work requiring special inspection is in
progress, The 81 shall document his/her observation, If the Special Inspector is
not performing his/her duties while work requiring special inspection is in
progress, staff must follow the steps listed below under "8",
8, All corrective actions must be properly documented (ie: correction notice and
computer entry including verbal warnings), Staff must follow the steps listed
below in the order shown:
Step 1, First occurrence
Written Notice - Performed by 81.
Issued by the 81 on site identifying the dutieslresponsibilities the Special
Inspector failed to comply with, The Notice must include a statement
informing the Special Inspector what are his/her duties/responsibilities as
it relates to the dutylresponsibility he/she failed to comply with, A copy of
the notice shall be given to the Special Inspector, Inspection Agency (if
applicable), contractor and the owner/owner's agent.
Step 2, Second occurrence
Second Written Notice - Performed by 81.
Issued by the 81 on site identifying the duties/responsibilities the Special
Inspector failed to comply with, The Notice must include a statement
informing the Special Inspector what are his/her duties/responsibilities as
it relates to the duty/responsibility he/she failed to comply with,_A copy of
the notice shall be given to the Special Inspector, Inspection Agency (if
applicable), contractor and the owner/owner's agent.

Step 3, Third occurrence
Office Interview - Performed by Snr, Inspector.
Snr, Inspector meets with the Special Inspector to discuss his/her
performance, code requirements and City requirements, and inform
him/her of the potential dismissal from inspecting projects in the City if
he/she continues to be irresponsible.
Step 4. Fourth occurrence
Dismissal - Performed by the Building Official or the Deputy Building
Official.
The Building Official or the Deputy Building Official, with the assistance of
the City Attorney's Office, notifies the Special Inspector and/or Inspection
Agency, in writing, that the Special Inspector is no longer authorized to
perform special inspection on projects in the City Of Chula Vista, The
special inspector or inspection agency may appeal the decision of the
Building Official orJhe Deputy Building Official to the Board of Appeals
and Advisors.

1. PURPOSE:
The purpose of this document is to inform special inspectors, inspection agencies, material
testing labs and HERS raters_hired for their services on building construction projects in the City
of Chula Vista of building code regulations and City requirements related to special inspection
and testing of construction material.

2, START WORK NOTIFICATION:
Prior to the first day a special inspector is scheduled to be on the job site, he/she must do the
following:

3, SCOPE OF AUTHORITY:
The special inspector's authority is limited to the extent of providing special inspections for the
types of work in the category in which he/she is certified for.
The special inspector is not authorized to:
A. Inspect or approve any work for which a building permit has not been issued,
B, Infer/inform others that the special inspection is in lieu of City inspection and approval.
C, Accept changes, revisions and substitutions to the approved permitted plans that have not
been approved in writing by the Building Division,
4, DUTIES AND RESPONSIBILITIES:
A. The special inspector shall observe the work assigned for conformance to the approved
permitted design drawings and specification,
B, The special inspector shall verify compliance with the requirements of the California Building
Code and other code related publications pertaining to special inspection, HERS raters shall
verify compliance with the requirements of the California Energy Efficiency Standards,
C, The special inspector shall bring all "discrepancies" to the immediate attention of the
contractor for correction, then if uncorrected, to the Building Inspection Section, the engineer
or architect of record, and the owner, The written report may be submitted by fax or delivered
in person,
D. The special inspector shall submit written inspection reports to the Building Inspection
Section, to the engineer or architect of record and to other designated person(s),
E, Prior to "Final" inspection, the special inspector shall submit to the Building Inspection Section
a final signed report (Form 4543 - Special Inspector/Inspection AgencyfTesting Laboratory
Final Report) stating that the work requiring special inspection is, to the best of his
knowledge, in conformance to the approved plans and specifications and the applicable
workmanship provisions of the California Building Code,
F. Prior to "Final" inspection, HERS raters shall submit a completed and signed "CERTIFICATE
OF FIELD VERIFICATION AND DIAGNOSTIC TESTING" (CF-4R) to the Building Inspection
Section,

5, SPECIAL INSPECTION AND MATERIAL TEST REPORT
For writing and submitting special inspection reports and for construction materials testing
reports, the following information must be included (not applicable to HERS raters):
A. The building permit number.
B, Street Address
C, Scope of work Performed, The reports must clearly and legibly state the scope of work
performed, The reports must also define the area of work inspected/tested by physical
location and/or grid lines, stories, etc" and the type of special inspection/testing performed
at each location,
0 Statement of Compliance The report must clearly state work performed "was in
compliance with the City approved permitted plans, details and specifications" or "was not
in compliance with the City approved permitted plans, details and specifications",
E, Date & Time, The reports must state the date of inspection and the time of arrival and
departure from the job site, Also the date the material was sampled and the date it was
tested,
F, Reports must be consecutively numbered,
G. Name & Signature with the certification number,
H, Report Submittal Date, The Building Inspection Section and the engineer/architect of
record must receive weekly inspection reports within five (5) calendar days after the date
when special inspection was performed.
The Building Inspection Section must receive material test reports within five (5) calendar
days after the date when testing was performed, A copy of material test reports shall also
be submitted to the architect/engineer of record,
I. Deviations When the special inspector becomes aware of a deviation from the Building
Code, the City approved permitted plans and specifications, or the standard construction
practice, he/she shall bring this to the attention of the project superintendent and/or to the
attention of the person responsible for doing the work immediately for corrective action,
If no corrective action is taken the special inspector must report such deviations or non-
compliance to the Building Inspection Section no later than the next business day by
calling and submitting a written report, The written report may be submitted by fax or
delivered in person,
6, FIELD REPORT(S) SHOULD INCLUDE BUT NOT LIMITED TO (EXAMPLE OF DETAILED
REPORTS): (Not applicable to HERS raters)
A. Concrete - Rebar grade, mix design, location of placed concrete, yardage, quantity of
samples, time spent in truck drum, slump, temperature of concrete, location of samples
taken, pumped or tailgated, consolidated, detail(s)/sheet numbers from approved set of
plans, batch plant ticket number, concrete supplier, lap and clearances and general
weather conditions,
B, Epoxy - Hole depth, diameter, evaluation report number, expiration and batch number,
location, approximate air temperature, detail(s)/sheet numbers from approved set of
plans,

C, Welding - Welders to be certified by name and qualified welding positions, verified
welding procedure specification (WPS), amps, volts, electrode(s), type of welds, location,
preheat, interpass, fit-up, detail(s)/sheet numbers, mill certification(s); changes to
procedure qualification record (PQR) may require re-qualification,
D, Structural masonry -location of C,M,U, wall, lift location, size of C.M,U" working stress
design, reinforcing steel for size, grade, lap, clearance, location, cleanouts, consolidated
and reconsolidate, location of samples - prisms, grout, mortar, grout supplied by, ticket #,
truck #, detail(s)/sheet numbers from approved set of plans,
E, Pre-stressed/post-tension concrete - see applicable requirements for reinforced
concrete, concrete reached a compressive strength of ~ PSI prior to tensioning
operations, gauge and ram number were used with a recent calibration date of ~,
tendons stressed to a gauge pressure of ~ PSI corresponding to ~ % of the
tendons ultimate strength and anchored at ~% of ultimate strength, individual tendon
elongations, detail(s)/sheet numbers from approved set of plans ..,etc,
F, Sprayed fireproofing - verify thickness, density and adhesion/cohesion strength of the
sprayed fire-resistive material (SFRM) in accordance with approved plans, specifications
and applicable ASTM standards, where a fireproofing schedule is not included in the
approved plans and specifications, prepare a schedule and submit it to the Building
Division, Document the names of SFRM applicators, Applicators shall be approved by
the manufacturers of the materials specified, unless specifically approved by the design
professional of record, Verify that the SFRM is as specified, verify that if primer is
required, it is of the type recommended by the SFRM manufacture Detail(s)/sheet
numbers from approved set of plans..,etc,
G. High strength bolting - verify type of joint specified (I.e, slip critical, non-slip critical)
bolts, nuts and washers are in compliance to project specifications, installation procedures
meet minimum bolt tensions required by code, faying surfaces are free from dirt, When
applicable, verify bolt tensioning torque with a tensioning device such as the skidmore
wilhelm at least once a day, once a shift and each time a new batch is opened, Bolts
other than the snap-off variety or the direct tension indicator shall have tension/torque
verified, detail(s)/sheet numbers from approve set of plans..,
7, SUB-CONTRACTING AND TESTING:
A. Special inspectors are strictly prohibited from employing, delegating and/or sub-
contracting their services to another special inspector after having been approved by the
Building Inspection Section to work on a project. Substitute Special Inspectors, temporary
or permanent, must be approved by the Building Inspection Section prior to performing
special" inspection,
B. Concrete cylinder test specimen sampling slump test and other field related work
associated with the concrete at the point of truck delivery may be preformed by an ACI-
GRADE 1 field technician, provided: the approved special inspector for concrete is on the
job site when the concrete truck arrives "and" the field technician's work is performed
under supervision of the approved concrete special inspector.
C, All construction materials and test specimen shall be tested by a testing laboratory listed
on the City of San Diego or the City of Los Angeles most current list of approved testing
laboratories. Testing laboratories are permitted to employ, delegate to and or sub-

contract their material testing services to another approved construction materials testing
laboratory,
D, Non-destructive testing of materials are considered to be testing and shall be performed
by qualified/certified technicians employed by and under the approved construction
materials testing laboratories supervision, Laboratories shall be on a current approved list
from the City of San Diego or the City of Los Angeles,
E, Special inspectors are strictly prohibited from performing non-destructive testing or testing
of construction material or assemblies unless they are employed by an approved
construction materials testing laboratory and are properly certified/qualified to perform
such testing,
F, The soils engineer or engineering geologist of record is deemed qualified as a special
inspector for excavation and filling, As an alternative, the soils engineer or engineering
geologist of record may designate another soils engineer or engineering geologist, or a
certified grading inspector, to be the special inspector,
8, CONTINUOUS vs. PERIODIC SPECIAL INSPECTION:
Continuous Special Inspection:
Continuous special inspection means that the special inspector is on site at all times observing
the work requiring special inspection,
Periodic Special Inspection:
Some inspections may be made on a periodic basis and satisfy the requirements of continuous
inspection, provided the periodic scheduled inspection is performed as outlined in the project
plans and specifications and approved by the Building Official.
NOTES:
A. If there is no periodic inspection schedule outlined on the project permitted plans, Special
inspections are required to be performed on a continuous basis,
B, Periodic special inspection will only be permitted to be performed for the types of work
which are allowed by the Building Code to be performed on a periodic basis in lieu of
continuous basis,
C, Periodic special inspections must include initial inspections, inspections of work in
progress in accordance with the schedule outlined in the project plans and final
inspections,
9, SUSPEND OR TERMINATE:
The Building Official or the Deputy Building Official may disqualify a special inspector, inspection
agency, testing lab or HERS rater from working on projects in the City Of Chula Vista when
he/she determines that the special inspector, inspection agency, testing lab or HERS rater is
incompetent or in violation of building code regulations and/or City requirements, A special
inspector, inspection agency, testing lab or HERS rater may appeal the decision of the Building
Official or the Deputy Building Official to the Board of Appeals and Advisors,

PLEASE READ THESE INSTRUCTIONS CAREFULLY. YOU ARE RESPONSIBLE FOR COMPLYING
WITH THESE REQUIREMENTS. ANY VIOLATIONS OF THESE INSTRUCTIONS MAYBE A CAUSE FOR
REMOVAL OF THE WORK PERFORMED.
Note: Throuohout this orocedure. HERS (Home Enerov Ratina Svstem) raters are to be considered saecial insaectors
and the resoonsibilities outlined below aDo Iv to HE'ìS raters as the'l aool'l to scecial inscectors unless noted
otherNise.
I. PROPERTY OWNER'S RESPONSIBILITIES:
The property owner or his/her authorized agent of record or the architec:/engineer of record acting as the property owner's
agent shall:
1. Complete, sign and execute the 'PROPERTY OWNER/CONTRACTOR A,GREEMENT' fcrm for the special
inspection, off-site fabrication or ccnstruction mater:als testing and submit it to :he Planning and Building
Department at time of permit issuance.
2. Employ one or more special inspectors who shall previce ¡r,s~ect¡cr,s during ccnstruction and fatricaticn of the
types of work requiring special inspection listed under Califcrnia Building Code !in the C2se of HERS raters.
Caliiornia Enerav Efficiencv Standards) as adcpted by the City of Chula Vista, See, 1701.5 and listed in the
approved plans/specifications and as required by prcduc: manufacturer's apprcval repcrts. These inspections are
in addition to and not a substitution for the required City inspections.
NOTE: The special inspection shall be performed by a spec:al inspector certified in their areas of expertise.
The ccnstruction materials testing shall be performed by a testing laboratorj listed on the City of San Diego or the
City oi Los Angeles most current list of approved materials testing laboratories.
The same special inspector(s) and the construction materials testing laboratorj shall be emplcyed during the
construction of the permitted building/structure to the extent that is possible.
3, Complete, sign, execute the 'PROPERTY OWNER'S FINAL REPORT FOR SPECIAL INSPECTION, AND
CONSTRUCTION MATERIALS TESTING' form and submit It to the Building Inspection Section prior to
requesting a final inspection,
This final report states that all work requiring special inspection, testing, or off-site fabrication was to the best of
his/her knowledge, performed in accordance with the approved plans, speoifications and the applicable
workmanship provisions of the California Building Code (in the case of HERS raters. California Enerav Efficlencv
Standards) as adopted by the City of Chula Vista
4. Remove and replace, repair, test and inspect all non-conforming work as proposed by the architecUengineer of
record and as approved by the City of Chula Vista without any cost to this jurisdiction,

II. CONTRACTOR'S/BUlLDER'S RESPONSIBILITIES:
The contractor/builder shall:
1, Complete, sign, execute the 'PROPERTY OWNER/CONTRACTOR AGREEMENT' form for the soecial
inspection, construction materials testing, off-site fabrication or HERS rating and submit it to the Building Division
at the time of permit issuance.
2, Notify the special inspec:or/construction materials testing labe-rator} and schecule to perform the insf:ection cf the
work requiring special inspection and/or testing.
NOTE: The special inspections are in addition to and not a substitution for the inspecticns required by the C;ty of
Chula Vista, Work performed shall not be conceaied untii it is approved by the special Inspector and the City of

NOTE: The fabricator of the building components shall be currently ceriified by ICBO, AISC, ACI, PCI, PTi cr
other recognized and approved certifying agencies. For ncn-certified fabricatcrs, or when required per the
approved permitted ~Icms and or spec:ficaticns, one or mere approved certified special inspectors shall be
assigned by the builoing owner when approved by the City of Chula Vista
8. Furnish the 'Certificate of Compliance for Off-Site Facr:caticn' form to the fabricatcr(sj cf building compcne:.ts to
be completed and submitted to the Building Inspection Section.
9. Keep a copy of all special inspection/test reports submitted by the special inspector and/the construction rT'.aterials
testing laboratory at the jcb site A copy of such reports shall be submitted to the Building Divisicn Inspec;;on

10. Correct all deficient and/or non-conforming work as noted by the special inspector/cons;ruction materials testing
laboratory in accordance with the approved permitted plans and specifications.

This form shall be completed and submitted by the property owner or his/her authorized agent of record cr the
architect/engineer of record acting as the property owner's agent and the contractor/builder at the time of
permit issuance,
APPLICATION TO PERFORM OFF-SITE FABRICATION
This application shall be completed by the fabricator of building components when such components are
fabricated within the premises of the fabricator's shop, This application must be submitted to the
Inspection Section for approval at least three business days prior to scheduled start of fabrication,

CERTIFICATE OF COMPLIANCE FOR OFF-SITE FABRICATION
This form shall be completed by the fabricator of building components when such components are fabricéted
within the premises of a fabricators shop, This form must be filed with and approved by the Building
Division Inspection Section prior to erection of fabricated components, Also see Application to Perform
Off-Site Fabrication,
INSTAllATION CERTIFICATE (CF-6R) and CERTIFICATE OF FIELD VERIFICATION AND
DIAGNOSTIC TESTING (CF-4R)
Form CF-6R to be completed and signed by the installer and form CF-4R to be completed and signed by the
HERS rater. Both forms must be submitted to the Inspection Section prior to final inspection.
PROPERTY OWNER'S FINAL REPORT
This form shall be completed by the property owner or his/her authorized agent of record or the
architect/engineer of record acting as the property owner's agent for the types of work required to have special
inspection, construction materials testing, off-site fabrication or HERS rating, This form must be filed with
the Building Division Inspection Section prior to final inspection,
SPECIAL INSPECTORIINSPECTION AGENCY/TESTING lABORATORY FINAL REPORT
This form shall be completed by the independent special inspector or the special inspection agency or the
construction materials testing laboratory after all work requiring special inspection or testing have been
completed, and must be filed to the Building Inspection Section prior to final inspection.
APPROVED CONSTRUCTION MATERIALS TESTING lABORATORIES
The construction materials testing shall be performed by a testing laboratory listed on the City of San Diego or
the City of Los Angeles most current list of approved materials testing laboratories,

Prior to the first day a special inspector is scheduled to be on the job site, he/she must do the
following:
1. Notify the Inspection Section of the project, date and approximate time he/she is
scheduled to be on the job site, and a phone number that he/she can be reached at
(preferably his/her cell phone,) This can be accomplished by either calling in the
information or completing this form and faxing it to the Inspection Section. This
information is needed to coordinate a time when the City inspector can meet the
special inspector on site,

Section.
The two requirements shown above are also required when a repiacement special inspector, other
