Home My WebLink AboutReso 1990-15619 RESOLUTION NO. 15619 RESOLUTION OF THE CITY COUNCIL OF THE CITY OF CHULA VISTA RATIFYING AN AGREEMENT BETWEEN THE CITY OF CHULA VISTA AND COMMUNITY CARE NETWORK (CCN) FOR THE PROVISION OF A PREFERRED PROVIDER HEALTH CARE NETWORK UNDER THE HIGH OPTION - JOHN HANCOCK HEALTH PLAN AND AUTHORIZING THE MAYOR TO EXECUTE THE AGREEMENT The City Council of the City of Chula Vista does hereby resolve as follows: WHEREAS, the City has been using and paying for CCN's preferred provider organization (PPO) as part of the High Option - John Hancock health plan since August 1, 1988, and WHEREAS, CCN discovered recently that they inadvertently did not enter into a formal contract with the City. NOW, THEREFORE, BE IT RESOLVED that the City Council of the City of Chula Vista does hereby ratify an agreement between the City of Chula Vista and Community Care Network (CCN) for the provision of a preferred provided health care network under the High Option - John Hancock heal th plan, a copy of which is on file on the office of the City Clerk. BE IT FURTHER RESOLVED that the Mayor of the City of Chula Vista be, and he is hereby authorized and directed to execute said agreement for and on behalf of the City of Chula Vista.
