Please read the followir~ information BEF RE completing the Board/Commission/Committee application. QUALIFICATIONS: While previous experience and education in the areas assigned to, the 1 •~ . Board/Commission/Committee are considered in the appointment process, your background is not the only determinant. Other interests, previous and current employment, and familiarity with Chula Vista and the community are also relevant. Furthermore, YOU MUST BE A RESIDENT OF THE CITY OF CHULA VISTA AND REGISTERED TO VOTE IN CHULA VISTA AT THE TIlVIE OF YOUR APPOINTMENT FOR ALL COMMISSIONS/COMMITTES EXCEPT YOUTH. PROTECT AREA COMMITTEES AND SOME EX-OFFICIO MEMBERS. A description of the functions and responsibilities assigned to each Board/Commission/Committee is available in the Office of the Mayor/City Council. If you wish to receive a copy, please ask the Secretary to the Council for this information sheet. Before applying, you should check the time and dates when the Board/Commission/Committee regularly meets and how much time you may be able to devote to this service. Each Board/Commission/Committee has a staff secretary and they will be glad to answer your questions regarding the time involved. If appointed to a Board/Commission/Committee, it is possible that you will be designated in the City's Conflict of Interest Code and be required to disclose some financial interests. Your principal place of residence in Chula Vista, real estate two miles outside of the city limits, and government bonds and mutual funds are exempt from reporting requirements. If you have additional other/different investments and financial holdings, information may be obtained from the City Clerk's Office or the City Attorney's Office regarding disclosure requirements. Conflict of Intermit -According to the Fair Political Practices Commission, all local ~~overnment agencies are required to adopt conflict of interest codes desienatin~_ certain positions which entail the making, or participation in the making, of decisions which may affect a Commission member's financial interests. The Commission to which you are appointed has been identified in Chula Vista's Conflict of Interest Code. Since you have already filed this year as a member of the Resource Conservation Commission, you are not required to the another form. This notice is simply to inform you that you are also roquired in the future to file one statCment but you must list the names of both commissions. Padilla ~~ ~~~~ ~~tl" City Of Chula Vista 276 Fourth Avenue Chula Vista, CA 91910-2699 Re: Resignation from Chula Vista Safety Commission Honorable Mayor Padilla: It is with great sadness that I submit this letter of resignation from the Chula Vista Safety Commission. It is unfortunate that due to my ongoing work commitments and travel schedule, I must step aside and ask that you find a qualified replacement to assist my colleagues in continuing to move the Safety Commission forward. I leave with a heartfelt gratitude and appreciation of my colleagues on the commission, who take time away from their families and busy schedules to make our wonderful City a safer place for all of us. Additionally, I would like to thank all of the representatives from the City's Traffic Engineering Staff and Police Department Traffic Division for providing a high level of professional support and educational guidance during my time as chairman. We have the finest engineers and police officers in the country and it is obvious that they are proud to represent our wonderful City. Finally, Mr. Mayor, as you are well aware, the Safety Commission is important to the citizens of Chula Vista and even more so, as we continue grow as a City and community, the efforts of the Safety Commission will be a vital component of that growth.
