Retired Police Officer of 15 years, having served two full terms on the Safety Commission (7 yrs as
chairperson}. Having lived in the city for all of my life, and serving on the police department, I have a treat
amount of knowledge regarding city history, processes, and politics. My service on the Safety Commission
was very satisfying in that 1 could use my traffic, legal and investigative experience to help bring a different
point of view to the public, members of staff and my fellow commissioners.
What would you hope to accomplish by your participation?:
I would like to brim that same knowledge, to the planning commission. I am known to visit sites, talk to
persons, and "do my homework" before the commission meetings so that I do not have to simply rely on the
staff reports and testimony from those present at the meeting. This way, 1 can be more educated to make the
right decisions for all of the citizens and not just the select few that attend the meetings. I would very much
appreciate consideration for the current opening and ask for an interview from the Council. Thank vou_ John

It is my pleasure, on behalf of the Chula Vista City Council, to extend our most sincere
congratulations to you on your appointment to the Safety Commission. Your term will expire on
June 30, 2010.
I will administer your Oath of Office during the City Council meeting of September 19, 2006.
At that time, you will also be given a Certificate of Appointment and a Boards and Commissions
Handbook. The meeting will commence at 6:00 p.m. in the Council Chambers, located in City
Hall, at 276 Fourth Avenue. If you are unable to attend this meeting, please call the City Clerk's
Office at 691-5041 to make alternative arrangements to take your Oath of Office, since this must
be accomplished prior to participating in Safety Commission meetings.
The Secretary of the Safety Commission, Lindsay Walters, will send you information regarding
Safety Commission meeting dates. Should you have any questions regarding the meetings,
please feel free to contact her at 691-5234.

It is with pleasure to inform you that the City Council on July 28, 1998 ratified your
reappointment to the Safety Commission. This means that you will be serving for another four
years. Your second term will expire on June 30, 2002. Because of term limits, you will not
be eligible for another term on this commission. You would be eligible to serve on another
board or commission if you so desired and had an application on file. No oath of office will be
necessary for those serving a second term.
Chula Vista is the great city that it is because dedicated citizens like you are willing to give of
themselves and of their time. The City Council is very appreciative of that fact. So, on behalf

During the City Council meeting of 7/12/94, you were appointed to fill a vacancy on the City's Safety
Commission. My congratulations on your appointment to this important commission.
The Oath of Office will be administered by the City Clerk during the regulaz City Council meeting on
7/19/94. It will be held at 6:00 p.m. in the Public Services Building, 276 Fourth Avenue. In the event you
are unable to attend this meeting, please call the City Clerk's Department at 691-5041 and she will gladly
make other arrangements with you so that you can take the oath.
The Secretary to this commission, Shirley Buxton, will send you information regarding the meeting dates.

Conflict of Interest -According to the Fair Political Practices Commission, all local government agencies are
required to adopt conflict of interest codes designating certain positions which entail the making, or
participation in the making, of decisions which may affect financial interests. The Commission to which you
aze appointed has been identified in Chula V'ista's Conflict of Interest Code and you aze required to file a
Statement of Economic Interest Form 730 upon assuming office (within 30 days of swearing in), yearly by
April 1, and a leaving office statement. Failure to file these documents with the City Clerk carries astate-
mandated fine of X10 per day. You may wish to review the blank or sample form prior to accepting office.
The City Clerk's Department stands ready to help you with the forms at any time.

QUALIFICATIONS: While previous experience and education in the areas assigned to the
Board/Commission/Committee are considered in the appointment process, your background is
not the only determinant. Other interests, previous and current employment, and familiarity with
Chula Vista and the community are also relevant. Furthermore, YOU MUST BE A
RESIDENT OF THE CITY OF CHULA VISTA AND REGISTERED TO VOTE IN
CHULA VISTA AT THE TIME OF YOUR APPOINTMENT FOR ALL
COMMISSIONS/COMMITTES EXCEPT YOUTH. PROTECT AREA COMMITTEES.
AND SOME EX-OFFICIO MEMBERS.
A description of the functions and responsibilities assigned to each
Board/Commission/Committee is available in the Office of the Mayor/City Council. If you wish
to receive a copy, please ask the Secretary to the Council for this information sheet.
Before applying, you should check the time and dates when the Board/Commission/Committee
regularly meets and how much time you may be able to devote to this service. Each
Board/Commission/Committee has a staff secretary and they will be glad to answer your
questions regarding the time involved.
If appointed to a Board/Commission/Committee, it is possible that you will be designated
in the City's Conflict of Interest Code and be required to disclose some financial interests.
Your principal place of residence in Chula Vista, real estate two miles outside of the city
limits, and government bonds and mutual funds are exempt from reporting requirements.
If you have additional other/different investments and financial holdings, information may
be obtained from the City Clerk's Office or the City Attorney's Office regarding disclosure
