It is my pleasure, on behalf of the Chula Vista City Council, to extend our most sincere congratulations
to you on your appointment to the Safety Commission. Your term will expire on June 30, 2013.
I will administer your Oath of Office during the City Council meeting of September 15, 2009. At that
time, you will also be given a Certificate of Appointment, Boards and Commissions Rules and
Guidelines, and a memorandum regarding AB 1234 Mandatory Ethics Training Requirements. The
meeting will commence at 4:00 p.m. in the Council Chambers, located in City Hall, at 276 Fourth
Avenue. If you are unable to attend this meeting, please call the City Clerk's Office at 619/691-5041 to
make alternative arrangements to take your Oath of Office, since this must be accomplished prior to
participating in Safety Commission meetings.
The Secretary of the Safety Commission, Florence Picardal, will send you information regarding Safety
Commission meeting dates. Should you have any questions regarding the meetings, please feel free to
contact her at 691-5234.

Dear Mr. Fuentes,
This email is to advise you Mayor Cheryl Cox has recommended to the City Council your appointment to the
Safety Commission. The City Council will confirm the appointment at the Council meeting on September 1st (please note
you do not need to attend the September 1st meeting).
Once the Council has voted to approve your appointment, you will receive a letter from the City Clerk's office confirming
your appointment and scheduling your oath of office for the September 15th Council meeting. The meeting will begin at 4
pm and the oath of office is done within the first 10 - 15 minutes of the meeting. If you cannot attend the Council meeting
on September 15th to do your oath of office, please contact Lorraine in the City Clerk's office. You may go to the City
Clerk's office anytime Monday -Thursday from 8-5 pm to do your oath of office, which takes about 10 minutes. Please
note, in order to participate as a voting member at the Commission meetings, you must complete your oath of office first.
In the meantime, should you have any questions, feel free to contact Lorraine in the City Clerk's office at 619-691-5041 or
