[ 4 ] CHULA VISTA CITY COUNCIL MEETING OF: February 4, 1975 ITEM TITLE: Resolution - Accepting bids and awarding contract for printed two way mailers INITIATED BY: Purchasing Agent BACKGROUND Bids were received and opened at 3:00 P.M. on January 27, 1975, in the office of the Purchasing Agent for furnishing one hundred fifty thousand (150M) printed two way mailers. The mailers will be used by Data Processing for the Sanitary Service billing. Proposal forms were mailed to three vendors with two submitting bids as follows: BIDDER UNIT PP.ICE AMOUNT Standard Register Company $ 85.93 M $ 12,889.50 Forms Management, Inc. 95.02 M 14,253,00 Funds required for the purchase of the mailers from Standard Register Company are $12,889.50, plus $773.37 sales tax for a total of $13,662.87. The City is to provide all necessary forms. It is requested that $13,622.87 be appropriated from the unappropriated surplus of the general fund and transferred to account #O1-1430-5212 for the pro~ect.
